包皮不一定要割，只有包皮过长的需要进行包皮环切手术。包皮过长指的是阴茎头被包皮完全包裹，但是可以用手翻开包皮，全部露出阴茎头。这种情况下如果包皮开口比较宽大，容易上翻，不需要进行包皮环切手术。如果包皮开口比较小，不容易上翻，或者是上翻后不容易回复，这种情况容易引起包皮嵌顿，所以需要进行包皮环切手术。
包皮长也不影响生活做爱，只是每天都有尿茧需要每天把包皮翻出来洗洗，保持干净卫生，做爱可能会敏感些，别的没什么，又不影响生孩子
